Enclosed are some forms that need to be filled out and sent back or dropped off before your scheduled day, so that we can get your information in our files. For future reference we will keep the Boarding Agreement, Boarding Applications and Veterinary Release forms on file so if you board at another time you will not have to complete those forms again. Also, please include your $20.00 deposit to hold your reservation. That money will go towards your pets stay with us. If you have any questions about the forms or deposit please give me a call. I appreciate your booking your reservation with Dakota Pets and we look forward to having your pet stay with us while you are away. |
